Creating Google Forms or Quizzes

Teachers can create Google Forms or Quizzes in their Google workspace accounts. To do this, please follow the directions below. 1. Log into your Google account and go to Google Forms.

2. Click Blank form to create a form or Blank quiz to create a quiz.

3. Title your form or quiz and add a description.
   
4. Select a question type by clicking the drop-down menu next to Multiple choice and selecting your desired question type.

5. Add a question by typing in the question in the Untitled Question field and setting up any additional information needed.
   
If you are setting up a quiz, you also need to click on the Answer key to set the amount of points the question is worth and to select the correct answer. Click Done once you have added this information.

If you want the question on the form or quiz to be required, toggle the Required switch ON. If not, you can leave this as OFF.
   
6. Click the button to add additional questions until you have completed creating your form or quiz. You can also import questions, embed images, insert videos, and more.

7. Once your form or quiz is completed, click on the Settings tab at the top.
8. Decide how you want to collect information about the individuals completing the form or quiz. If you want to collect email addresses, choose Verified or Responder input. Verified will record their email if they are logged into their Google Accounts when completing the form or quiz. A message will appear on the form or quiz that states their email addresses are automatically being collected. Responder input will give them a place to type in their email address.
  
9. Read through and adjust any of the other settings for your form or quiz. For quizzes, pay close attention to how you want questions presented and when you want grades released.

10. Click on the Send icon in the top right.

11. Click the link icon and then check the box next to Shorten URL. Click Copy.

12. Share that link with the individuals who will be completing it.
